FHSINDX - Changes to Index File Creation program

• 08/08/98 - changes to support Long Place Names
• 04/02/98 - corrected "error 05 at line 58560" when creating index file using SORTSEQ system table and sort key field contains a null character (ASCII 0 character)
• 12/24/97 - corrected "error 05 at line 1240" that occurred for invalid birth/death dates which were negative or had month>255
• 10/01/97 - added instructions for optionally using SORTSEQ table for defining the sort order for characters of text fields
